1 Completed Employment Application
2 Color copy of your Social Security card
3 Color copies of your valid Driver's License (front/back)
4 Copy of your Car Insurance Card
5 Copy of your Registration
6 Copy of your current CPR card (if you have one)
7 TB skins test; to be done at your local health department or your primary physician's office
After Interview, the office will give you the needed documents to do your Fingerprints/Background Check
6 Two (2) copies of fingerprints with last page of background paperwork: To be done at your local sheriff's office

Additional forms will be added to your file after you are hired This is done by the Office Manager or RN Supervisor for Home Care Specialist's in the Mountains, Inc. If there are any questions regarding your packet, the office will contact you for clarification. Keep all receipts for reimbursement.
